T-Mobile - Unauthroized Purchases

GROTON, CONNECTICUT -- So we finally cancelled our insurance on T-Mobile, for the third time. Mysteriously it keeps coming back on our bill with no reason why. Everytime we have it cancelled we don't get the full refund back because it was prorated. Yet because it wasn't on the bill statement, we aren't ever offically covered by T-Mobile. So how can it be prorated if it was never offical?
The run-around we get on this is frustrating on a continuous basis. Also we have Unlimited 2Gig internet, yet they think its ok to "upgrade" us to 5 Gig and charge us extra for it, yet we use a few hundred Megabytes.
