Routine USCG Drug Test - Complaint

Made an appointment for 11:00 am to have routine USCG 5 point drug screen per USCG instructions. Receptionist led me back room and asked for my ID, insurance and script, provided. I told her I was there for a standard 5 point drug screen per the script and showed her the USCG form and instructions. Like I wasn’t even there, or hadn’t listened to me, she handed the script to a male colleague and said “Call the office and find out what they want” Without being rude, I repeated that I want the standard 5 point drug screen test and again showed her the CG 317P Form, and attendant instructions.
She wasn’t interested, it was her way or no way.
Was then escorted to test room and told to wait by another woman who said they were phoning. I again showed her the USCG Form and instructions, which she took away and brought back in 6-7 minutes. She then advised me that the test could not be done because I did not have a ‘Custody and Control’ form. Again I showed her the instructions from the USCG, indicating that only the form I had was required. She refused the test. I got up, retrieved my paperwork and walked out, stopping only to tell the receptionist staff member that I hoped she wasn’t as rude and condescending to all the customers who went there as she was with me.
Returned to my office and called QD Customer Service 866-697-8378 and relayed events to Vivian, who apologized and added that that is not the way clients are supposed to be treated! (Really?) She promised to send info to Office Manager there who would call me to resolve within 24 hours, and if no call back, I should call back to Customer Service.
