Innovative Health Management Complaint - re: "Patient Responsibility Invoice" Acct. 17172
LAS VEGAS, NEVADA -- Dear Dr. Kristensen and Associates:
This letter is being submitted to a complaint database with regard to your erroneous billing from November 2000 of $90.00 to us instead of our health care provider as of 7/20/1999 (PacifiCare - Southern Nevada). We were informed by PacifiCare that the reason this claim was never processed by your office is due to the fact that it was never received FROM your office.
This is the SECOND time your office has failed to submit a claim on our behalf properly -- the first was for a visit in November 1998, which you attempted to bill my wife for this past March (2000). Apparently your staff member (male; no name given) "adjusted the billing off" at that time instead of reporting it to her health provider of record at that time (not PacifiCare; prior to our marriage)... for whatever reason. In any event, it's not our problem.
Please be advised that, without an immediate and final dispensation of this matter, copies of this letter will be distributed to all applicable federal, state and local agencies which govern the practice of medicine in Las Vegas and the licenses which your office currently holds. This may include but is in no way limited to the following: American Medical Association, Nevada State Board of Medicine (or its equivalent), Office of the Attorney General (State of Nevada), and any physician review boards with jurisdiction in your area. Furthermore, preparation for filings related to punitive damage claims will be initiated until this matter is resolved.
We are seeking an immediate cease-and-desist of all billing related to this matter, including a corrected statement reflecting no balance owed. You are hereby notified of having 30 days to respond from this date (22 January 2001) or we will seek further legal action as recommended by counsel.
