ABERDEEN, WASHINGTON -- First off, I'd like to comment about Aflacs' customer service. I'm an agent for Aflac, and I call the same toll free phone number that policyholders call. Not once have I not had my call answered. I call them at least 4-10 times a week; as I file all my policyholders claims for them. I'm their voice ears and feet when it comes to claims and I love it. Secondly, I just printed off a re-instatement form for a client and for the record the accident re-instatement form is 5 pages long. All I did was fill out the name address and policy number, asked the policyholder the only question there is on the actual application which is pertaining to participating in any activity for a wage or compensation. Then had the policyholder sign, then I faxed it. You may have gotten the universal reinstatement form, BUT if you look over the form, find the cancer section, check the box and answer the few questions, wham...you're done.
I've searched high and low for the 30 page re-instatement form you are speaking about; no such luck. I apologize for your confusion, and wish you knew of an agent who would have helped you through this. If I'd of heard about it, I'd of been glad to.
