RE: TV DVR XXXXXXXXXX
To Whom It May Concern:
Upon the advice of legal counsel I am offering TV Inc. this final opportunity to take corrective action. Please be advised this certified – return receipt letter will be my FINAL attempt to close the above referenced TV account.
In January 2009 our TV unit died and we opted to use our cable systems DVR option. A call was made to end the TV service. Since that time TV Inc. has ignored all telephone and written requests to stop service on the non-functional TV DVR (XXXXXXXX) and has harassed me by email and U.S. Postal Service and threatened damage to my credit rating by turning the account over to collections. This is unacceptable and as a customer I have the absolute right to end my service at anytime without harassment.
To avoid further action on my behalf you must comply with the following:
a. Close the account (XXXXXXXXXX) per all previous written, verbal and this final request
b. Credit the account back to January 2009
c. Forward a paper statement by U.S. Postal Service reflecting the corrected ZERO balance
d. A statement from TV Inc. that this matter is now closed and no further collection actions will be taken on account (XXXXXXXX)
Your affirmative response in this totally unnecessary matter is awaited.
